To update or correct your name, civil status, mobile number, email address, or residential address, you need to submit the required documents to Maxicare via email. For name and civil status changes, send the necessary documents to enrollment@maxicare.com.ph. For updates to your mobile number, email address, or residential address, email customercare@maxicare.com.ph.
The CIF requires members to provide personal information, enabling Maxicare to maintain accurate records and deliver efficient services. It also complies with the "Know Your Customer" (KYC) policy, essential for identity validation in financial transactions. Completing the CIF is a one-time requirement, but updates should be made if there are significant changes in your personal information.
Maxicare offers an Online Payment facility where you can settle your bills using various payment options, including credit and debit cards (Visa and Mastercard), e-wallets (GCash and GrabPay), online banking (BPI and UnionBank), and installment plans (Metrobank and BPI) for transactions of at least PHP 8,000. To access this facility, visit the Maxicare website, navigate to 'Services,' and select 'Pay Existing Plan.'
An LOA is required for consultations or procedures at Maxicare-affiliated hospitals that aren't Primary Care Clinics. You can request an LOA through the Maxicare Member Gateway portal or by contacting Maxicare's Customer Care Hotline.
All active Maxicare principal members or dependents aged 18 and above can register on the Member Gateway. You'll need your Maxicare Card number, a unique email address, and a mobile phone number to create an account. If you don't have your physical Maxicare Card yet, you can still sign up using your card number.
